Melwin Joseph, DO
Dr. Melwin Joseph is a graduate of Marian University College of Osteopathic Medicine in Indianapolis. He completed his residency at Ascension-Macomb Hospital in Warren, Michigan. He specializes in primary care, diabetic care, and cardiovascular health. In his spare time, he enjoys staying active, traveling, and spending time with his family.
Geraldine Ntum, FNP-BC
Geraldine graduated with her BSN and MSN from Loyola University and Northern Illinois University, respectively. She is a skilled nurse practitioner with 19 years in healthcare and 5 years of NP-relevant experience assessing, diagnosing and treating patients of all age range with acute and chronic illnesses while advocating for patients’ needs and providing proficient care. She serves as the President of Cameroonian Women Associations in Chicago (CAMWAC). Geri is also bilingual, proficient in English and French. “What motivates me every day to give the care that I provide is the love I have for the profession, and I think compassionate care is important because we have to be able to understand what the patient is going through in order for us to give them the care that they need.”
Allison Curran, MSN-FNP
Allison earned her BSN from Bradley University and MSN-FNP from Loyola University Chicago. She sees adults and children (ages one year and up) and works in women’s health. She enjoys spending time with her family, exercising, and baking.
Melisa Morales, MSN-FNP
Melisa received her MSN-FNP from Loyola University in 2005 and is board certified by the American Academy of Nurse Practitioners. Melisa has over 21 years of nursing experience. She has experience in primary care services and women’s health.
Niraj Shah, MD
Dr. Shah is a graduate of Rush Medical College in Chicago and completed his internship and residency at Rush University Medical Center. He is board certified in Internal Medicine and serves as medical co-director at The Parc at Joliet Nursing Home. He specializes in primary care, preventative services, and weight-loss counseling. He enjoys basketball, trying new and exciting foods, and spending time with his children. “Being in Internal Medicine, I aim to build trust with each of my patients. This is the foundation that allows me to achieve each patient’s individualized healthcare goals.”
Danielle Ratazak, MSN-APN
Danielle earned her MSN degree at the University of St. Francis. “I went into nursing to make a difference and help people. I started as a nurse in the ICU for seven years and then went back to school to further my career. What I enjoy most about my job is being able to see patients get better and provide them with education, as well.” She has been married for 11 years and is a mom of 7-year-old twins and an 8-year-old. She helps coach baseball, loves traveling to Florida, and is actively involved with her kids’ school activities. Danielle also enjoys attending paint & sip events and going to vintage fairs.
Lisa Smith, MSN-APRN-FNP-C
Lisa attended Lakeview College of Nursing and Lewis University to obtain her BSN and MSN, respectively. She is board certified by the American Academy of Nurse Practitioners. “I enjoy getting to know each of my patients on a personal level and giving them the best individualized care and treatments to help them excel.” Lisa enjoys spending time with her husband, children, and dog. She loves to travel, cook, and exercise.
Mustafa Aladin, MD
Dr. Aladin graduated with high honors in 2015 from the American University of Antigua College of Medicine. He completed his residency in combined Internal Medicine and Pediatrics (Med/Peds) at SUNY University at Buffalo in Buffalo, NY. He was also a chief resident during his last year of residency. He enjoys playing cricket and is an avid Chicago Whitesox fan! “I chose to practice Med/Peds so I can be a versatile physician caring for both children (Pediatrics) and adults (Internal Medicine) and serve my local community, especially with its growing population.”
Emily Lawrence, FNP
Emily graduated with her BSN and MSN from the University of Alabama at Birmingham (UAB). “I became a Nurse Practitioner because I enjoy building relationships with patients and empowering them to take an active part in their medical care.” During her free time, she enjoys watching Chicago sports and camping with her husband, sons, and two dogs.
Yatin Shah, MD
Founder of Primary Care Joliet, Dr. Yatin Shah, has been practicing for over 28 years, including in the Joliet area since 1989. He is board certified in Internal Medicine. He is medical director of Sunny Hill, The Parc of Joliet, Joliet Terrace, and Frankfort Terrace Nursing Homes, as well as a member of the Illinois Board of Pharmacy.
