3) How do I get my medication refilled?
• It is always best if you request your medication refills during your routine office visits.
• You may call the office and follow the options to the triage nurse. You may speak to or leave a message for the nurse about your medication. Please have the name of your medication with the dosage and directions available including the name and phone number of your pharmacy.
• You may request your pharmacy to fax your refill request to your physician.
4) What is the difference between a Physician and a Nurse Practitioner?
A Nurse Practitioner is an Advanced Practice Nurse (APN) who is licensed in all states to provide high-quality health care services similar to those of a physician. Nurse Practitioners can serve as a patient's primary health care provider . All Nurse Practitioners are Registered Nurses who have completed extensive education and training. Most have a masters or doctorate degrees. They have provided excellent health care for more than 45 years. Nurse Practitioners are licensed through the state nursing boards. In addition, they must pass a national board certification exam in their area of specialty. Dr. Yatin Shah has a collaborative agreement with all Nurse Practitioners at Primary Care Joliet, which means that he is available for consultation and emergency care.
Nurse Practitioners are able to diagnose and treat acute and chronic conditions such as high blood pressure, diabetes, asthma, joint pain, infections and injuries. They are able to perform physical exams, order and interpret diagnostic tests, prescribe medications and other treatments and make specialist referrals.
Nurse Practitioners focus on health promotion, disease prevention, wellness and patient education. They practice in many health care settings which include clinics, urgent care centers, hospitals, and nursing homes to name a few.
5) How do I get a Referral to a Specialist?
You must be seen by one of the Providers at Primary Care Joliet. After an exam is completed and a course of treatment discussed, if a referral to a specialist is needed, the Provider will then submit the referral request. Once a referral is processed you will be informed and a referral will be made available to you. Once you receive the referral you may make an appointment with that specialist.
6) If I have an emergency, what should I do?
• If you feel it is an extreme emergency you should call 911 or go to the nearest emergency room.
• Primary Care Joliet has two Urgent Care Facilities available for quality acute care. (Check Locations).
• Primary Care Joliet has a Physician available on call 24 hours a day 7 days a week for emergency situations.
7) What if my family member only speaks Spanish?
Primary Care Joliet has several staff members and a Physician that is fluent in Spanish. They are very helpful and understanding of the needs of the Hispanic patient.
